Abstract Guidelines for Nominating Aim ONE "Best Practices"

It is important that all CCHF members have a chance to identify and nominate existing exemplary programs. We use the term "program" broadly to include any methods, ideas, activities, services or outreach operations within a health center or other ministry that address issues of providing care for the underserved. Your nomination could be anything from a simple and effective method of providing transportation to a description of how your health ministry began. Examples of Best Practices include:
- a church and parish nurse program
- a unique health promotion poster
- a description of the significance of longevity among providers in a rural practice
- a prenatal outreach program
- a home-bound elderly care program
- a model of how to integrate faith into health care

We would like to identify effective faith-based programs that meet these criteria:
1. The program should have its roots/genesis in and draw on the vision and values of a clear Christian approach to health care.
2. The program should be designed as a part of wholistic health care in an underserved poor community.
3. The program should address needs that are widely present in underserved poor communities, i.e., it should be potentially replicable in numerous other communities.
4. The program should make evident the effect of integrating faith within the health care context.
5. The program should improve access to health care, demonstrate progress toward reducing disparities in health status between minority populations and the total population, or otherwise demonstrate measurable improvements in significant health indicators.

Are there particular programs in your health care ministry that meet these criteria? Programs that you would like other CCHF ministries to know about and perhaps duplicate in their communities? It may be helpful for you to consider broad areas of your ministry such as: a) things you did in starting a health care ministry; b) activities/programs which are a part of operating your ministry; c) things you are doing to improve health care activities and health outcomes; and d) programs addressing the needs of specific sub-groups of your population.

Guidelines for a brief (1 - 2 page) abstract:

Take into consideration all of the above criteria please provide the following:
1. Background and description of the issue or need to which the program is a response.

2. Brief program outline -- what you are doing. Include how faith is a dimension of the program.

3. Any information about outcomes/results.
