2019 Conference - Collaborating with Pharmacist to Extend Clinical Services in Chronic Disease Management

Catherine Harrington.We will explore the collaboration story between the Pharmacy School at Palm Beach Atlantic University (PBAU) and the Community Health Center (CHC) of West Palm Beach, an indigent care clinic started by the First Baptist Church. PBAU pharmacists staff the Chronic Disease Clinic under a collaborative practice agreement with the CHC medical director. PBAU takes referrals of patients with chronic disease (e.g. diabetes) needing intensive management and the Chronic Disease Management Clinic works with those patients to meet their medication, emotional, and spiritual needs on a long term basis.

Beth and Greg Delaney.In a recent survey over 1/3 of CCHF clinics reported that they plan to expand scope of services to include MAT (medication assisted treatment). This session will look the trends in MAT via information gleaned from recent meetings with SAMHSA, ONDCP, and DHHS, and will share about faith-focused recovery providers who are exploring, and/or using MAT.

The role of the pharmacist extends far beyond the hospital basement and pharmacy counter. Although the value of the pharmacist is often recognized in a pharmacy, many are unaware of the pharmacy services that can be provided in a clinic setting to both patients and other healthcare providers. Several studies are cited at, American Association of Colleges of Pharmacy documenting the benefit of pharmacy in conjunction with primary care showing that these patients were more likely to have medication relationed issues addressed, including non-compliance and untreated indications. This session will feature real-life examples of pharmacy integration.
