David Chen.We will examine the diverse contexts of firearm injuries such as unintentional pediatric injuries, suicide, intimate partner violence/domestic abuse, and other criminological/assault types. We will also discuss the evidence on the effectiveness of injury prevention counseling for each of these groups as well as current legal mechanisms for harm reduction and how these have been used.

Mark Dal Corso.This session will briefly describe the concepts of Community Oriented Primary Care (COPC) and Community-Centered Health Homes (CCHH), and describe how public health students have created systematic ways for patients to learn about community resources, empowering them to improve their health outcomes. The participants will engage in an interactive exercise to develop "bridges" from the clinic to the community in which they are working. 

2019 Conference - Collaborating with Pharmacist to Extend Clinical Services in Chronic Disease Management

Catherine Harrington.We will explore the collaboration story between the Pharmacy School at Palm Beach Atlantic University (PBAU) and the Community Health Center (CHC) of West Palm Beach, an indigent care clinic started by the First Baptist Church. PBAU pharmacists staff the Chronic Disease Clinic under a collaborative practice agreement with the CHC medical director. PBAU takes referrals of patients with chronic disease (e.g. diabetes) needing intensive management and the Chronic Disease Management Clinic works with those patients to meet their medication, emotional, and spiritual needs on a long term basis.
