2019 Conference- Case Management: A New Frontier with Free Mobile Clinic

Rachel Bardgett.Good Samaritan Health Services is a mobile medical unit that has been in operation for 20 years. The standard staff members include paid positions for a nurse, driver, and 2 providers for the daytime clinics with volunteer providers for our evening and weekend clinics. The sharp increase in the growth of our clinics and of more complex, complicated patients made clinic more challenging with only one nurse to handle everything from triage to order processing, and patient education. We have implemented a RN Case Management position to assist in increase in compliance, chronic disease management, and statics tracking.

We will share scriptures and principles to answer those who question choices you make to glorify God that are counterintuitive to the world's economy.

2019 Conference - Practical Firearm Violence Prevention for Every Clinician

David Chen.We will examine the diverse contexts of firearm injuries such as unintentional pediatric injuries, suicide, intimate partner violence/domestic abuse, and other criminological/assault types. We will also discuss the evidence on the effectiveness of injury prevention counseling for each of these groups as well as current legal mechanisms for harm reduction and how these have been used.

Matthew Allen. Dr. Allen will also share how the fellowship helped him leave his role as dental director of a federally qualified health center and embark upon a year-long sabbatical.

2019 Conference - Building Bridges from Clinic to Community

Mark Dal Corso.This session will briefly describe the concepts of Community Oriented Primary Care (COPC) and Community-Centered Health Homes (CCHH), and describe how public health students have created systematic ways for patients to learn about community resources, empowering them to improve their health outcomes. The participants will engage in an interactive exercise to develop "bridges" from the clinic to the community in which they are working.

2019 Conference - Collaborating with Pharmacist to Extend Clinical Services in Chronic Disease Management

Catherine Harrington.We will explore the collaboration story between the Pharmacy School at Palm Beach Atlantic University (PBAU) and the Community Health Center (CHC) of West Palm Beach, an indigent care clinic started by the First Baptist Church. PBAU pharmacists staff the Chronic Disease Clinic under a collaborative practice agreement with the CHC medical director. PBAU takes referrals of patients with chronic disease (e.g. diabetes) needing intensive management and the Chronic Disease Management Clinic works with those patients to meet their medication, emotional, and spiritual needs on a long term basis.

Beth and Greg Delaney provide a simple but helpful look at the SCIENCE behind addiction. They Introduce the importance of Connection/Belonging for recovery, the importance of collaborative networks of care and community engagement, and identify faith's role in the continuum of care for SUD. They will give insight as to faith's role and opportunities in prevention, intervention and recovery supports.
